  The name of the supplier.

• Email

  The email address of the supplier’s contact person.

• Address line 1

  The address of the supplier.

• Address line 2

  Additional information about the supplier’s address, if necessary.

• Address line 3

  Additional information about the supplier’s address, if necessary.

• Address line 4

  Additional information about the supplier’s address, if necessary.

• Address line 5

  Additional information about the supplier’s address, if necessary.

• Name contact

  The name of the supplier’s contact person.

• Phone number

  The contact person’s primary phone number.

• Website

  URL to the supplier’s official website.

• Account manager

  The name of the account manager in your company responsible for managing this supplier.

• Business Registration Number

  Government-issued registration number for the supplier organization.
